A toothpick would actually be less sanitary because it's porous. And I seriously doubt that a barista or anyone serving coffee is going to use hand tools to decorate the drinks; this person wipes between each dip, if you notice. Even if it was a screwdriver, you never use anything that's going near food and drink on anything BUT food and drink. So it'd be no different than using a fork or a knife.
